*Nose: slight hump nose, crooked nasal bone, deviated septum, rhinitis. My nose wasn’t ugly, wasn’t a piggy nose, and was a triangular nose. *After surgery: slight hump refinement, crooked nose refinement, septum/rhinitis surgery, nasal tip support strut using the removed septal cartilage, septal cartilage placed on the nasal tip, AlloDerm on the nasal dorsum *Hospital where I had surgery: a hospital famous for revision surgery (I chose a hospital famous for revision surgery to minimize side effects, and one that could also address ENT issues)
*Problems after the first surgery (open approach) 1. My nose looked long because of the septal support strut. 2. After placing septal cartilage on the nasal tip and adding a septal support strut, my nostrils showed (it wasn’t like that at first, but my nostrils became more visible over time) 3. The side profile was not smooth because of absorption of the AlloDerm on the nasal dorsum (it was okay at first, but the shape gradually changed) 4. As the nasal tip drooped, the side profile looked slightly like a witch nose when smiling. (I wanted a semi-upturned nose, but it didn’t happen)
*Revision surgery again after 6 months (open approach again): I originally wanted everything removed.. but I fell for the doctor’s persuasionㅠ To improve the 4 problems above, they would refine the uneven nasal dorsum and place the remaining AlloDerm back on the nasal tip. Why? Because the nasal tip had drooped.. They said the reason my nostrils showed was that if the drooped nasal tip were raised a bit more, they wouldn’t show.
*Problems after revision surgery None of it improved + because of inflammation in the columellar scar from opening twice, the surgical scar was obvious to anyone.+ The left side was slightly pinched, making the nasal bridge look crooked.
*Third revision surgery (remove everything): closed approach, free of charge. I also went to many consultations at other hospitals (about 5), but the prices were expensive and everyone only said it would be difficult, so I had it done again at the original hospital. They left only a very thin layer, about one layer, of the roughly 3 layers of AlloDerm placed on the nasal dorsum. Because they said if it was all removed, the nasal dorsum would become bumpy..
*3 months after removal The long-looking nose improved. The slight pinching on the left improved. The nostrils showing improved; it’s not back to my old nose, but I think it has returned about 65%. The open-surgery scar,, the inflammation has gotten better, so it’s gradually improving. The nasal dorsum looks smooth, but feels bumpy when touched. I sometimes think maybe I should have removed all the AlloDerm, but they say it will be absorbed anyway, so I’m just accepting it. I could originally make a slight piggy nose even after surgery. I can do it after removing everything, but it still doesn’t become as unattractive a piggy nose as before surgery. If I press my nose firmly, it presses in slightly, but it doesn’t flatten as much as before surgery. After removal, the columella didn’t sink all the way in; it went in by about the amount of the septal support strut that was removed, but not as much as before. There is no upturned-nose phenomenon. I was prescribed Singulair for a month because I was worried my nose would turn up, but I don’t know whether it worked or not.
*Conclusion: I’m glad I removed it. I regret not just removing everything during the second surgery. I regret that I ended up with an open-surgery scar. For the AlloDerm on the nasal dorsum, I’ll just wait for it to absorb.
*Hospital: It’s a bit... hard to share the information, but to give a hint, it was originally an ENT clinic, but it kept getting bigger. It absorbed a facial contouring hospital, absorbed a dental clinic too, and keeps expanding. It’s famous for revision surgery, and I think it was good that I had everything removed at the hospital where I had originally done it instead of going elsewhere. Other people seem to come out pretty just fine. ㅠ I think the reason the shape didn’t turn up or the columella didn’t sink in after removal is that I didn’t originally have a piggy nose or a low nose, so the nasal tip manipulation wasn’t severe (though they still did everything that counts as nasal tip manipulation).
