Please give me advice on revision surgery for my nasal alars..

It’s been 15 years since I had surgery where they tied the nasal tip, raised it with ear cartilage, and reduced and lifted the nasal alars from the inside. I don’t like the shape of my nose, but I can live with that. The problem is that my nasal alars look really strange.. In the photo, the areas that look dark as if they’re folded also look shadowed in real life, almost as if they’re crumpled, and even from the front, the lower part of the nasal alars looks thick and folded. It’s like they just stitched the ends of the nasal alars inward as if folding them in.. On top of that, they lifted the left side more, so it’s even asymmetric. At the time, I hated the idea of scarring from alar reduction, so I went around to many places, and only one place said they could do it with that kind of novel method, so I had it done there. There was a reason everyone else didn’t use this method... I went to 6 places, and 5 of them didn’t even understand what had been done to my nasal alars. Even after I explained it, they said they couldn’t tell by looking. Some places said, “Weren’t you originally shaped like this?” or “To me, you just naturally look asymmetric.” It seems like it’s such an uncommon surgical method that they can’t even get a sense of it. Three places suggested solutions. Two places said we should vertically excise the nasal alars to reduce the thickness. They said it’s not a commonly used method, but it seems like the only option. The problem is that other hospitals discouraged this method, saying the scar would be large and it would be functionally risky. Only one place figured out how the surgery had been done and even pointed out the asymmetry. I wanted to remove the nasal cartilage and redo the revision surgery, but they said I should restore the nasal alars first, and cosmetic surgery would come after that. They said if the nasal alars are restored, the nostrils may look a bit larger than they do now, but they won’t reduce them at the same time, so it’s right to decide after seeing the result. (My nostrils weren’t originally large, so I’m worried they might become too small.) Just hearing the explanation, this seems the most reasonable, but other hospitals said this method is impossible because skin had already been removed and sutured. This doctor also said we’d only know after trying, and that they couldn’t guarantee it. They refused to vertically cut and reduce them, saying they personally won’t do that because it’s dangerous. Even if I want to go to more consultations, when I go, the doctors can’t even diagnose what kind of condition it is, so no direction comes out.... I’m also deliberately using my monthly leave to go, and I don’t want to keep putting it off forever... I thought about a university hospital too, but since it’s not contracture, I don’t think it’s a major-hospital case either.. I don’t think I’ve seen a case like mine.. Could anyone recommend a hospital worth consulting at or give me some advice?
