Got my surgery date (+ clinic consultation review)

Original nose: hump nose + arrow nose Surgery: shaved down the hump a little, used autologous costal cartilage + septal cartilage to lift the tip + surgery for rhinitis and a deviated septum Skin is on the thin side (the side of the tip used to get red and then settle down / the cartilage can be a bit visible and palpable) < No major problems in daily life> I just do not want to spend the rest of my life anxious, and I want to get rid of this bomb that could go off at any time, so I hope to have everything removed. At the consultation, I had clearly decided on autologous costal cartilage + surgery for rhinitis and a deviated septum, but I only found out 2 years after the operation that septal cartilage had been used in the tip. Looking at the operation record, mesh was used too. I wondered if mesh was used because it had to be fixed as a rib graft, but I do not really know. According to the manager, if there is plenty of rib cartilage material, why would they need to use mesh... Anyway, since I am going for full removal, there is no mental shock. I went through full-removal reviews on a community and another forum, searched through posts all the way back to 2020, watched YouTube too, and did a lot of research, narrowing it down to a few places. Once I filtered out the obviously negative reviews, only a few places were left. I went to two consultations in person. Both said full removal was possible. 1. The price was roughly 500 to 700, an easy operation around difficulty 1/10 After checking the CT and operation record, they confirmed there was a step deformity, so they had filled it with rib graft material. If a step deformity or tip hollowing is seen after full removal, dermis would be used (AlloDerm/autologous dermis). Rhinitis and deviated septum surgery was recommended (optional). Dissolvable sutures were used (I do not know what they are used for). Since this is not an inflamed nose, there is no need to remove the capsule. Just like the consultation reviews I had seen, the style was very confident. I am not particularly concerned about scars, so if dermis were used, I would prefer autologous dermis. But we all have a certain level of distrust toward doctors, right?... So I decided on autologous dermis, but I also worried that they might use something else. I looked it up and saw posts saying dermis is hard to remove later, and that sort of thing, which made me think dermis can also cause problems, so I passed on it. 2. The price was roughly 420 to 530 Honestly, I wanted to have surgery here, so I went in secretly hoping they would say full removal was possible... I had heard they were very safety-oriented, so... Anyway, they said the surgery used only autologous cartilage, so there should not be any problem (they seemed not to notice the mesh in the record?) and asked why I wanted removal in the first place. Still, I clearly said that I wanted full removal for the following reason. They said it is rib graft material, so 100 percent cannot be removed. That means it would end with some rib graft material still left, but as the doctor above said, it is my own cartilage, so it should be fine... In any case, everyone knows that completely shaving everything away is difficult, right? Because the skin is thin, if the rib graft material is shaved down, the skin will be damaged more. This phenomenon where the skin turns red and then settles down may not go away, and it may even get worse, so make sure you understand that before deciding. If the hump has to be shaved, they would shave it a bit more, so the nose would become lower. The tip would be refined a little, so it would be lifted more than before. (They showed before-and-after photos, and since I had an arrow nose, that actually worked in my favor.) Because this nose involved the septum, if full removal causes the center of the nose to sink in (saddle nose), they would take ear cartilage and use it during surgery. They explained that in the end, it means I came to remove cartilage, but cartilage might be used again. They also said this is not a nose that needs to be built up with dermis. This is only my guess, but I think the mesh may have been the only real issue. But my anxiety is very severe, so... I decided to remove everything. During the manager consultation, they said that sometimes the nose looks fine during surgery, but only after all the swelling goes down does saddle nose become visible. People who cannot tolerate that get revision surgery, while people who think it is not a problem just keep living with it. They also said a drooping nose can droop more, and an upturned nose can become even more upturned. I decided to have surgery at the second clinic and have already finished the date and deposit. I just do not want a nose that has to be hidden under a mask all the time. Please, if it can just be a human nose, I hope it turns out well I should take a lot of photos before surgery.. I think I will cry a lot on the day of surgery.
