It’s a nose with no implant, supported by ear cartilage + a mesh graft, but the skin has gotten thinner so the tip is showing roundly, and I’m thinking about removal or revision surgery... Removal clinic 1: on palpation + ultrasound, the support stays as is, remove the ear cartilage used for the mesh graft and rearrange the cartilage; even without adding dermis, it’s fine at this level; if removed, the skin will get thicker; it seems like it has to be done before winter. Removal clinic 2: on palpation, the support stays as is, remove the ear cartilage used for the mesh graft and rearrange the cartilage; if needed, additional ear cartilage can be harvested; even without adding dermis, it’s fine at this level; since it’s your own cartilage, it won’t protrude, so you actually don’t have to do it, but if you’re worried, you can. Revision clinic 1: CT + palpation, remove the ear cartilage used for the mesh graft, rearrange the cartilage, and you absolutely have to add dermis; it has become too thin and pointed, so dermis must be added unconditionally. The CT wasn’t useful because there’s no implant, and they only looked by palpation, but they said the columellar support doesn’t seem to be there. They said it had become too thin and I came too late, so dermis definitely has to be inserted. What should I do??? Is it better to add dermis? I’m worried that if I don’t add dermis, it’ll be too thin and might indent or wrinkle They said it would get a bit thicker anyway, and that adding it unconditionally isn’t always better... But I am still worried... If the removal clinic says it’s okay, maybe I don’t need to worry...
