Remove everything from the nose and lower the columella?

I’m writing this cautiously because I’m worried and wanted to ask for advice. First, if you look at my previous post, you can tell what my nose is like right away. My goal is to remove everything. But my natural columella is already tucked inward. To explain it simply, it feels like a sunken columella? A doctor at another hospital described it in a similar way. The rankings in my head keep going back and forth between GY S and SY. SY’s basic stance was, “Remove everything,” and there was nothing extra they said to do. They said if the bridge lowers and I feel a loss, Megaderm could be used to supplement it. That was the end of it. GY S said, “Remove everything! But since the columella is that way by nature, if you lower the columella part when removing it this time, satisfaction will probably be better.” That was the end of it. At the moment, my mind leans a little more toward GY S than SY, but because they specifically pointed out the columella area, I’m torn. The places I consulted talked about my congenital columella area, but they never said anything like, “Let’s do something about this one,” so... So the conclusion is that I currently have silicone in the bridge and ear cartilage in the tip, and I’m wondering whether I should do full removal together with lowering the columella. If I lower the columella, are there side effects or other problems? If I do lower the columella, they said since the right ear cartilage was already harvested once, they would take cartilage from the left ear.
