The doctor was very kind. If the patient doesn't want it, he doesn't recommend it unnecessarily. My philtrum is quite long, but I dislike the narrow width and the downturned corners of my mouth more, so I only mentioned those, and he said we should just do those. But later when I said my philtrum was bothering me too, he agreed that it is on the long side and said he also thought it would be good to do it later. It seems like he doesn't make recommendations first unless the patient says something.
