Manager -> doctor -> manager in that order. I received an explanation from the manager about the surgery method, etc., and then went into the doctor's room. Both of them had basic courtesy. Since the doctor already had a surgery schedule, I waited after the manager's consultation. Fat grafting would still have some effect even if I did not do it, but because the hollows were deep, fat grafting was recommended for a better result.
