Hello.! About 15 years ago, I got swept up in the mood with my friends and had male rhinoplasty done together with them, but now my nose is quite crooked, so I’m looking into revision surgery.
I’ve been doing research and am currently making consultation appointments at about 3–4 places, and I’ve already had consultations at 2 of them.
[1st consultation clinic] - I was recommended nostril lowering + autologous dermis (buttocks). - I had a CT scan, but maybe because the machine quality was not good, an accurate analysis was difficult. - According to the director, it was confirmed that the nasal bridge is significantly crooked, but it was difficult to determine whether what is inside is silicone or to assess the current condition. - Also, since my nostrils are quite visible for a male nose structure, they suggested lowering the nostrils, correcting the nasal bridge, and then raising the bridge with autologous dermis. - Since this is revision surgery, they said it would be better to take a safety-focused approach, and that the actual condition cannot be known precisely until the surgery is opened up. In particular, they strongly opposed using costal cartilage, saying that other clinics would probably recommend it but it could bend again.
[2nd consultation clinic] - I was recommended costal cartilage + custom-made 3D silicone. - The CT scan results came out very accurately, and I was able to receive a relatively detailed explanation. - They said my current condition is that the nasal septum and nasal bridge are significantly crooked, and the existing silicone is tilted toward the right sloped side due to my facial bone structure and the crooked nasal bridge. - On the CT, it was also confirmed that the silicone itself had deformed into a crooked shape. - The surgical direction was: Nasal tip: correction with costal cartilage + nasal bridge: stable placement using custom-made 3D silicone (They also suggested correcting it so the nostrils are less visible.) - Regarding autologous dermis or silicone removal, they explained that since I have maintained about 3 mm of silicone for 15 years and the skin has stretched, removing it could lower the nose and may lead to more wrinkles as I age. Also, based on the fact that there is no inflammation even though the current silicone has shifted, they judged that I likely do not have a major constitutional issue with silicone. - They also confirmed internal swelling due to allergies and recommended functional nasal surgery together.
I still have additional consultations left, but I’m worried about what overall direction I should take.
1) Whether to go in a safety-focused direction centered on autologous dermis 2) Whether to go in the direction of using costal cartilage and custom 3D silicone
While thinking about it alone, I found out about this community to get information, so I’m posting here.
For the consultation itself, I trusted the director at the 2nd clinic much more, and I liked that they treated me very sincerely and attentively. But the 1st clinic told me that costal cartilage should absolutely not be used for my nose, so I’m worried my nose might become crooked again...
I would be truly grateful if anyone with a similar experience or any advice could share.
