I wrote a post below telling people to ask me questions, and many people sent me private messages... But most of the questions were the same, so I'm writing this here. I am someone who had osteotomy surgery for a retruded chin 6 months ago. I will write what I felt and what I know about osteotomy for a retruded chin. 1. Is osteotomy for a retruded chin the easiest among facial contouring surgeries? -> It may be the easiest among facial contouring surgeries.. but it is absolutely not an easy surgery. That is closer to something clinics say to entice patients. Osteotomy for a retruded chin is a difficult surgery that requires cutting the muscle and operating while carefully avoiding the nerves. The chin bone also has to be cut beautifully.. and left-right balance has to be matched.... It is absolutely not an easy surgery. 2. Before having osteotomy for a retruded chin... please judge carefully whether your chin is actually small or not.. (the vertical length of the chin), or whether your chin is not small but your mouth protrudes, making the chin look relatively set back. If the chin is not small but the mouth protrudes and makes the chin look retruded, the results of osteotomy for a retruded chin are often not good. Think about it. If both the mouth protrudes and the chin protrudes, what would the face shape look like... And if the chin is long, the mouth is not protruding, and the chin is severely retruded, the length of the chin bone should be reduced while bringing it forward. 3. When doing osteotomy for a retruded chin, if the very end of the chin is cut and moved forward, the result would be much better... but if the end of the chin bone is cut, the cut bone dies.. So they cannot cut the end of the chin bone (the very bottom part of the chin), and instead cut the middle of the chin bone. In other words, it is not just the chin tip that comes forward; the middle of the chin comes forward too. Doctors with a lot of skill and experience appropriately shave down the protruding part in the middle of the chin. The clinic where I had surgery seems not to have known even this. 4. When fixing the cut chin bone, they usually use a titanium fixation plate, tie it with wire, or use an absorbable fixation plate. 1) Absorbable fixation plate -> Because of strength issues, the cases where it can be used are limited. 2) Titanium fixation plate -> It is used the most because the surgery is easy. But.. usually the fixation plate is used in the center of the chin.. Then, as mentioned above, it becomes difficult to properly shave the middle part of the chin, so it is easy to end up with a strange chin shape where the middle of the chin bulges out. A chin looks pretty when the chin tip protrudes slightly... And later, once the bone heals, there is the inconvenience of having to remove it. It has to be removed so the bulging in the center of the chin goes down... 3) Wire -> They say it is the best in terms of chin shape after surgery. But it is difficult to use. They say it is hard to match left-right balance. They say it requires a lot of skill from the doctor. 5. Side effects of osteotomy for a retruded chin and retruded chin surgery 1) Facial fat decreases. -> It differs by individual and by the doctor's skill, but... many people say they lost a lot of facial fat and look older. As the chin comes forward, the skin is also pulled tight.. and the cheeks often look hollow. 2) If the chin tip or the central part of the chin comes forward... the middle areas at both sides of the chin tip... that is, the areas below both corners of the mouth... look relatively sunken. This makes a person look quite older. Think carefully.. if the chin tip and the center from the front come forward, but the area where the chin and cheeks meet is hollowed out, what shape would that be... 3) Lip sensation issues.. -> At both ends of the middle of the chin, that is, the areas below both corners of the lips, there is one opening on each side where nerves emerge. Osteotomy for a retruded chin inevitably requires cutting the muscle just under the lip at the front of the chin... Also, for the surgery, when the area between the lower gum and lip is incised and the tissue is spread, the nerves mentioned above are inevitably stretched a little. That is natural. The tissue has to be spread so that a saw or laser device can go in.. go in and cut the bone.... At this time, the nerves may receive slight damage. And since the muscle under the lip is also cut.. of course the muscle nerves will also be damaged... So for a while, sensation in the lips disappears. Clinics usually say this recovers 100%, but... Among people who have experienced osteotomy for a retruded chin, some do not recover 100%... and recovery takes a long time. I am also 6 months post-op... but my sensation is still not complete. The general opinion is that it usually recovers 90-95%. They say if sensation does not recover even after about 1 year after surgery, you have to give up. There are also awful doctors who try to brush it off by saying, "It doesn't really interfere with your life, does it?" The nerve area is an important part that depends greatly on the doctor's skill. 4) Skin problems -> Many people look at facial contouring surgery too lightly.. In the case of osteotomy for a retruded chin, the chin swells a lot. After the swelling subsides, the skin often loses elasticity, with many wrinkles forming in the skin. It gets a little better over time, but... it is extremely stressful. My skin was something I never felt inferior about anywhere... but one of the things I regret most after retruded chin surgery is that my skin got worse. I wrote what came to mind first. If I think of other things, I will post again. One thing I want to ask is... please do not take retruded chin surgery too lightly. There are people who succeed, but there are also many who fail. Osteotomy for a retruded chin requires considerable precision from the doctor... and compared with how much the impression improves if it succeeds, the damage if it fails is far too great. Please think very carefully... Only consider it when orthodontic treatment or fat grafting truly will not work. At least if it is an implant, it can be removed if it does not work out.
