Considering revision surgery for visible nasal tip cartilage due to thin skin ㅠ

I had surgery 8 years ago using silicone for the nasal dorsum and ear cartilage for the nasal tip, but recently the ear cartilage in my nasal tip seems to have shifted to one side, drooped, and become visible through the skin. At this point, should I have it removed? For people with thin skin and visible nasal tip cartilage, how did you get revision surgery? Please recommend hospitals ㅠ My skin is so thin that even the silicone on the nasal dorsum looks slightly bluish and the tiny blood vessels show through. Should I have all the silicone removed, or would it be better to lower the silicone a little and replace it? Please recommend ㅠ
