Medpor removal

I had surgery 12 years ago. Because of nasal bridge reduction, silicone, and insufficient ear-cartilage support at the nasal tip, I had surgery using an artificial septal Medpor implant. 8 years ago, at another hospital, I had only the nasal tip revised; I remember it as cartilage repositioning at the nasal tip with AlloDerm added as reinforcement. Up to now, it has maintained well without contracture or deformity, but the Medpor inside my nose is slowly trying to come out. My original nose was small and slightly upturned, and because I also had double-jaw surgery 6 years ago, my nasal septum is probably in a state where part of it has been cut away. I’m wondering whether I should remove everything from the nose, or only remove the Medpor and silicone and fill the remaining area with costal cartilage…
