Should I remove the columellar strut or not

I’m on a pinocchio nose right now. I’m going around for consultations and even the doctors are shocked... It’s my original nose, low and bulbous.. so I didn’t even know my nose could ever be brought out to something like this in the first place. Autologous costal cartilage with silicone... the bridge area has 7 mm of silicone. The nostrils are in a 11 shape, and the upper part is almost attached, giving a water-drop feeling. It’s hard to breathe, and when I inhale deeply, the alar area sticks tightly and I can’t breathe. The first surgery didn’t include a columellar strut, and the nose was so soft that you could hardly tell it had been operated on. The silicone was low and it was so natural that no one knew it was a surgery nose, but even then the bulbous tip wasn’t fully corrected so it was blunt. Still, that was better, and I’ve been suffering so much from the foreign-body sensation and hardness from the autologous costal cartilage that I’m thinking about removing the columellar strut. But they mostly recommend leaving it in Is there anyone who removed the columellar strut and regretted it? Also, could anyone who left the columellar strut with ear cartilage tell me what the foreign-body sensation was like? My original nose also had a slightly tucked-in columella
