A: From the first surgery, it was done wrong. Remove the silicone from the nasal bridge and either rearrange the autologous ear cartilage at the tip, or harvest new cartilage and graft it. Use septal cartilage as well. Remove some of the scar tissue and capsule. They said there was an infection and contracture. B: They said contracture had occurred, but did not mention infection. Remove the silicone, remove the existing ear cartilage at the nasal tip, put in new ear cartilage at the tip, lower the tip, and use a reverse-rotation technique (?). They did not mention the capsule (I didn't even think to ask). C: They said it looks too much like an avatar nose, and that this kind of thing is a chronic inflammation nose. So the entire capsule has to be removed, and because the skin is too thin, if the whole capsule is removed the uneven tissue underneath will show, so autologous dermis has to be added no matter what. Remove the ear cartilage at the tip, harvest new cartilage, and insert it (if the existing cartilage can be reused, they will do that too). If the tip is lowered, it will look wider, and because the existing nostrils are on the large side (they said they are really quite big), they recommend alar reduction as well.
