Inflammation after nose surgery

Hello... My first surgery was on 2025-01-20, using ear cartilage + Gore-Tex. There was no inflammation at that time, but I was dissatisfied with the shape. Then on 2025-08-20 I had a second surgery with 4 mm silicone and autologous costal cartilage for the tip, at a different hospital. After surgery, starting from the two-month mark, the silicone would swell, and I have been taking antibiotics whenever it swells. At first there was a one-month gap, then once a week, and today I got very swollen on Saturday, so I kept taking them without stopping, but I’m still swollen now. At the hospital I went to, they said to try non-open nasal irrigation, and if that doesn’t work, remove it, but I really don’t know much... 1. If I just remove it, will contracture occur? I currently have no signs of contracture. 2. Can they remove only the silicone and still correct the reconstruction right then during surgery? 3. Has anyone here had irrigation done... Did it get better T_T 4. Please recommend a hospital. This is so hard T_T
