Temporalis fascia + silicone + ear cartilage combination
1. Because it is a closed surgical method, precise surgery is impossible compared with an open approach -The silicone still moves, and when I had a CT scan, its position itself was crooked
2. Thin skin was also a factor, but because of capsular formation, inflammation kept coming and going -It turns red at random times (it doesn’t hurt, and it isn’t contracture yet)
3. A dramatic line is impossible -Other people think I just got one line of nose filler (this could be an advantage for someone, but not for me)
I recommend this kind of surgery only to people who are operating on the nasal tip alone. (Actually, I don’t recommend it. I recommend making a columellar incision and operating while seeing it accurately with your own eyes.)
Everyone else, do it the standard way. Don’t regret it like I do.

0) Surgery clinic I had surgery at a place considered one of the originals. I heard it’s hard to even make a reservation there now?
1) Didn’t you get aftercare/revision? At the time, they didn’t even take a CT and judged only by sight/touch, saying too much cartilage had been injected and that it would be fine if they shaved a little off, so I had surgery again with a closed approach (took 20 minutes) I was stupid too. I should have at least whined and asked them to take a CT, but I trusted them completely -> While going around for consultations for this revision surgery, I got CT scans and found out the silicone itself was crooked
2) Where did they place the temporalis fascia? They said they placed it at the glabella so the silicone wouldn’t show through, but what nonsense, it had no effect at all. Only the temporalis fascia scar remained It seems like unnecessary harvesting and surgical material
3) The silicone moved even almost 2 years after surgery They gaslit me so much, saying crap like “silicone can move,” and since there wasn’t major contracture or inflammation at the time, I accepted that part
4) Because the cartilage is crushed before being inserted, more has to be harvested For this revision surgery, I used autologous costal cartilage, but the doctor said that if needed, they might harvest ear cartilage to make the nasal tip round. But after surgery, they said there was a ton of ear cartilage in my nose, so they reused it (Oh, for reference, both ears had been used^^)
5) Because it is closed, no splint is needed / recovery is fast After trying both, if they touched the bridge and dorsum, a splint is essential (I’m now 3-4 weeks out from my newly done nose, and the silicone doesn’t move at all. I was gaslit so much that I thought all silicone moved) Aside from the columellar incision area, the recovery period is the same. There is no difference at all (Of course it would be different if osteotomy is involved, but who would do osteotomy with a closed approach?)
6) Why I recommend it only for the nasal tip How are you supposed to make a columellar strut with closed cartilage injection? It’s just placing cartilage on the nasal tip. So I think it’s a surgical method that can be satisfying without side effects only when you’re touching up the nasal tip very slightly
I got the surgery because I absolutely didn’t want an incision scar, so I obsessively searched only for closed approaches. But I keep thinking, what if I had searched more carefully and had consultations at more varied places?
While looking into this revision surgery, I also felt that unlike 2 years ago when I had mine done, there are now so many more clinics doing closed + cartilage injection, and I thought people would be tempted like I was
This is something everyone says, but really, really do a lot of online research, go to lots of consultations, and think hard! And unless you’re someone who has keloids, or someone who is reeeally only raising the nasal tip slightly, I absolutely do not recommend it. So if you think your nose isn’t that kind of nose, don’t even look at this method
