Nose Re-Surgery

What is best for revision surgery if inflammation developed after rhinoplasty with autologous costal cartilage?

My first surgery used septal cartilage and silicone. For the second and third surgeries, I had revisions using autologous costal cartilage and silicone. A few months after the third and supposedly final revision, the rib cartilage at the tip was reabsorbed, the columella contracted and became depressed, and inflammation developed inside the nasal tip. Pus began accumulating. Drainage with a syringe and antibiotics did not help, so I underwent surgery to remove the inflamed tissue. The inflammation has now disappeared. Nine years have passed since then. I have no current inflammation, but because the columella is depressed, my nose is very small and low (1 mm of silicone was inserted). Looking at my nose is extremely stressful, so I am considering revision surgery. I am afraid that autologous rib cartilage could be reabsorbed again and cause inflammation, and I dislike the idea of another scar because I have already had rib cartilage harvested twice. If I were guaranteed 100% success, I would have it harvested again, but since that cannot be known, I am considering donor rib cartilage or other surgical methods. For those who developed inflammation after using autologous rib cartilage, what material did you use for revision surgery? Is there a hospital that could handle a case like mine well? T_T
