This is an extreme nose that sits between cosmetic and reconstructive. Most clinics said it would be hard to improve, and the places that said it was possible said they'd have to open it up first to know, and that I'd have to give up on some cosmetic goals or have surgery split into 2-3 stages. But I couldn't accept that or admit it because of all the hard time that had passed and the future time still ahead of me, so I kept denying reality. There were a lot of things to take into account, and a lot of time had already been wasted, so I shouldn't have rushed, but I had my own standards, so after clearing my mind and deciding on a minimal flap, with a light heart I looked mainly at the shape and line, and then went to the consultation without much thought. There were many reviews about contracture and reconstruction, and especially noses with shapes similar to mine, and according to the doctor and the manager, they told me about cases where even more extreme noses had been perfectly corrected, which changed my mind. My columella had already once undergone necrosis, and even though it had been composite grafted, they said my tissue condition was better than expected and my skin stretched well, and they told me about a positive surgical plan. After only hearing negative things for so long, hearing that it was okay made me secretly happy, but then I got caught up in wondering whether it could really be done or whether it would just become a repeated vicious cycle. They also explained how they would handle it if the columella went wrong again, so after a lot of thinking and worrying, I decided to trust them and go ahead.
