When I said I wanted the columella lowered like a Western nose, he said if done, it would lower the columella and also bring it down, but it would be safer to change the direction of the alar base a bit. They said the surgery would involve putting me to sleep for about 10 minutes, and during that time they'd do anesthesia and proceed with the cosmetic surgery. They said they'd make about 9 nose silicone pieces and try at least 2 before inserting them. They said they'd pay close attention to hemostasis for safety. From the review photos, the change wasn't dramatically extreme.
