The cosmetic consultation was the most systematic. They did not recommend osteotomy for the hump nose and said to just smooth it a little (they said my septum was already weak, so we should not overwork the nasal bone). They said fistula surgery is possible, but there is a risk of tearing more during the procedure, so in that case they would leave it alone and use only costal cartilage. The success rate for a nasal fistula is about 50%, and since it is enough if even one of the two nostrils is blocked, they said the surgery differs for each nostril. Lowering the glabella (such as removal) can make the nose look slimmer, and saddle-nose correction is possible. General anesthesia is a little concerning.
