A step-off if only the nasal tip is left

I’m thinking of removing the silicone and leaving only the autologous costal cartilage in the nasal tip, but they say ear cartilage has to be used to fix the step-off. But I really don’t want to use it. Do I absolutely have to use ear cartilage to fix this step-off? If only the nasal tip is left, does a step-off always occur? Isn’t this something that, depending on the doctor’s skill, can be done without using it? I’m only thinking of silicone removal and lowering the nasal tip height to match my original nasal bridge. If I use ear cartilage, I have to pay an additional fee too, and I don’t want to harvest ear cartilage.
