To keep the nose from collapsing after removal, do the alar cartilages need to be supported?

I'm planning to have surgery to remove 25 mm silicone and septal extension, so I consulted about five clinics. At two of the clinics, they said that if I only have removal done, there is a possibility that my nose could collapse later, and that there are cases where people eventually need revision surgery again. So they said that, for the nasal shape, it would be safer in the future to support between the alar cartilages with ear cartilage. They said it supports from the inside, so there is no risk of show-through, the nose stays soft, and you can still make a pig nose. They also use only a small amount of ear cartilage. Some of the three other clinics said they would just remove it without using ear cartilage ...
