Septal extension, 5 mm silicone

I had septal extension surgery, and the silicone is about 4 to 5 mm. The philtrum area keeps catching and the foreign-body sensation is severe, so I’m looking into removal. 1. Eight out of ten hospitals say that for the shape to come out, I need to have some kind of support, even if it’s not an extension strut but a floating support. 2. Two hospitals say that having any form of support will be uncomfortable, so they would remove it, tie the cartilage, and try to create the shape as much as possible by placing cartilage on top. I’m torn. The silicone inserted is also thick, so if I remove everything, I feel like the sense of loss will be too big. I don’t like dermis because the final thickness can’t be predicted, and at the university hospital they said silicone is not something you look at twice, and ㅡ ㅡ I don’t know what to do, my head is about to burst...
