Question About Cartilage Repositioning After Complete Removal

In the past, I had L-shaped silicone, my nose was built up with septal cartilage, and ear cartilage was used for the nasal tip. Now a lot of the cartilage shows through at the tip of my nose, and when I'm tired, the lower part of my nose feels tight. On severe days, even my front teeth feel a bit numb and tingling. Among the hospitals I looked into, one said they would definitely remove everything completely and cleanly, but they didn't mention cartilage repositioning. It felt like they would operate with only the functional aspect of the nose as the top priority. In this kind of case, is cartilage repositioning not essential? My original nose had a fleshy, spread-out nasal tip. I wonder if cartilage repositioning would make it look more spread out aesthetically...
