Does anyone know the difference between nasal tip cartilage repositioning for cosmetic surgery and nasal tip cartilage repositioning for restoration?

Even when doing nasal tip cartilage repositioning for cosmetic surgery, don’t they separate it from the tissue around the cartilage before doing it? Since they open the nose for that too, the cartilage would get weaker too, so I’m confused. When removing implants, why do they try to leave the support strut? People who only had repositioning for cosmetic surgery don’t have collapse and are doing fine, so couldn’t we also be okay if we remove the support strut and just do the repositioning well? Is it because opening it once and opening it twice are different? I feel like I’m saying something really pointless. Sorry.
