I'm considering a third surgery after revision surgery. Please give me advice

I know I should show a full-face photo, but I would appreciate it if you could give me advice based on this photo alone.. Other people might say, "This is a nose that has already been operated on twice?" but my nose was originally very broad and unusually shaped. During the first surgery, silicone was inserted all the way to the nasal tip, and as time passed, it became completely visible through the nasal tip, so this time I had revision surgery at another hospital. But instead of changing the structure, they only tied the cartilage together, or did something like that, and I was told the cartilage at the nasal tip had broken into pieces. Four years have passed and I have had surgery again, but honestly I feel like going back to that hospital and demanding compensation. That was a separate story. If you look at the photo, the columella is also hanging down, and the nostrils are slightly uneven.. Since I had a skin graft on the columella too, they did not touch it because of blood circulation concerns. In the photo, my nose does not look that broad, but when I look at myself in the mirror, it looks broad. So I kept wondering why, and as you can see in the photo, the boundary of the nasal ala is not a C-shape curving inward like other people's, but straight; on the left side, the upper part even looks wider. When I used to look at other people, everyone's nasal ala area looked distinct, so in a case like this, I don't know whether I need to have another surgery to reduce the nasal width or what I should do. It must have been hard to read because this is long, but I would be grateful for any advice
