3 years ago, I had nose surgery with autologous costal cartilage + septoplasty + 4 mm silicone. 6 months later, the nasal tip dropped. I was told it could be revised with ear cartilage, but I said no because harvesting my own rib cartilage was too painful. I had thought the autologous costal cartilage would hold it in place, so I still don't know why the tip dropped,... I wonder if it might have been a mistake by the doctor, but I don't know. 1. revisit the original hospital ㅅㅅㅇ (see another doctor at a different clinic) - CT showed silicone shifting and possible fibrosis, and possible absorption of autologous costal cartilage - cannot use the existing autologous costal cartilage. Need new autologous rib cartilage or donor rib cartilage - use thinner silicone - open rhinoplasty 2. ㅇㅂㅈ - said that CT is not needed if no functional rhinoplasty is done - asked who the previous surgeon was - existing autologous costal cartilage cannot be used. Need autologous or donor rib cartilage - 1–2 mm silicone - open rhinoplasty 3. ㄴㅈㄹ - took CT, suggested it would be good to do septal deviation surgery together - explained by sketching the shape on a printed photo of my face - said a new autologous costal cartilage is better - said existing autologous cartilage may have calcification - said because silicone starts from the level of the glabella and passes over the skull bone, it would be better to do it quickly - closed rhinoplasty 4. ㅊㄷㅇ - no CT, said the existing autologous costal cartilage can be used, asking, “Isn’t it a waste?” - said even if it is calcified, they can cut it and use it, and if the condition is good, use it - because ear cartilage is being used and the skin is thin, suggested using AlloDerm - non-incision alar base narrowing can be done or not done - closed rhinoplasty I visited four hospitals in total. I want to reuse my own autologous costal cartilage... I don’t want to harvest again, and I don’t want donor rib cartilage either.
