The doctor was extremely kind. If the patient doesn’t want it, they don’t forcefully recommend it. My philtrum is quite long, but I disliked my narrow horizontal width and downturned mouth corners more, so I only mentioned that, and the doctor said we should just do that. But later, when I said the philtrum bothered me a bit too, the doctor said it was true that my philtrum is on the long side, and later said they had also thought it would be good to do it. It seems they don’t make recommendations first unless the patient mentions it themselves.
