4 years after nose surgery, looking into removal and revision

My nose was originally my biggest complex, but now I think the surgery was a good choice, even though I keep getting these obsessive worries again and again.... My natural nose was small and short, with a high bridge but a slightly broad shape, a low and short tip, basically a small, short nose. I wanted to get rid of that complex, so I looked into a no-implant option, but on my doctor's recommendation I had a little bit of bone shaved off the bridge, a thin silicone implant placed, and the tip raised. I went in after barely researching anything, just because a friend recommended it, so maybe my own needs weren't understood well either.. I've been struggling with obsessive thoughts for 4 years... But recently I started thinking that the shape of my nose changed, and that brought even bigger obsessive worries, so every day I can only think about my nose and keep staring at it until I feel like I'm going crazy. The changes are: first, as the skin on my nose got thinner, the silicone and cartilage became too visible, and my nose became crooked. It was crooked before too, but now it leans in a different direction... And the height is different,,, the left tip cartilage has sagged, but the right tip cartilage is visible so it looks higher... I feel insane.. So I want to remove the silicone and adjust only the tip to a height that matches my bridge without letting it collapse, but I really.. have no idea where to go.
