Silicone removal after 20 years, bulbous nose, and deviated septum surgery!!

Implant-free, osteotomy, septal cartilage for the nasal tip><Day 1><Day 10><Day 10.><Day 10>I had surgery for a bulbous nose 20 years ago.. I thought they would make my large, chubby nose smaller and sharper, but at a plastic surgery clinic in Apgujeong back then, whose name I do not even remember, they made my already large nose even bigger. They had inserted 5 mm silicone to make my chubby nose look sharper, but when I saw the result, I sobbed. It was way too high. If I lay on my side, I could probably set a whole dining table on top of my nose. I went to the hospital crying and saying it was too big and too high, and that it was not what I had wanted, so just one week after surgery, I had another surgery to replace it with 3 mm silicone. The doctor was very apologetic and said this was the best that could be done. He said the lowest silicone had been inserted, and there was nothing else that could be done. Looking back now, from that doctor’s aesthetic perspective, I think he probably thought 5 mm silicone would look really pretty because of my high forehead and operated that way. Anyway, I lived with a big nose, thinking I was not meant to have a pretty nose in this lifetime. But in 2025, the tip of my nose started aching with a sore, twinging sensation. It was not extremely painful, but the sore feeling kept continuing in an unpleasant way. So I compared it with past photos, and wasn’t my nostril becoming triangular? Contracture had developed. After being fine for 20 years, it happened in just a few months. I do not know the reason. Even though I had silicone inserted and surgery done, I had bumped my nose here and there quite a lot, and I had also had Rejuran, alar Botox, and so on, so I cannot know what the cause was. As I started thinking that I needed revision surgery, I became intensely desperate to remove the silicone inside my nose. Since I myself am also a doctor (though my specialty is far from plastic surgery or ENT), I asked around among people in related specialties for places that were known to be good. They mentioned Cobijou, ㄷㄴ, ㄷㅍㄹㅅ, April, ㄷㅇㅇ, ㅎㅇㅇㄷ, and so on. Among them, I liked Cobijou the most because the review photos showed a beoseon-like, pointed, and not overly high nose, and I also heard recommendations that the director’s hands were on another level. With a nervous heart, I made an appointment and had a consultation, brought photos that I had Photoshopped into the shape I wanted, and kept asking questions. At first, the director listened to everything, but later he seemed to get fed up. I kept asking the same questions again and again... I get it..ㅜ Anyway, the director said we should not make it high, should reduce the size, and should make it natural, and since that matched my opinion, I decided to have surgery here! On the day of surgery, I arrived at 9:40 a.m. and was discharged at 3:40 p.m., and I think the surgery took about an hour and a half. In addition to the planned silicone removal, he said there was Medpor in my nasal tip, so it was difficult to remove it. He told me that using my deviated septum, he aligned the nasal tip and even the nostrils, and it turned out prettier than planned. He did not use ear cartilage either. Up until the surgery, it was easy. It did not hurt either. From the night after surgery: Hell. I was someone who slept breathing 100% only through my nose, with zero mouth breathing, so while sleeping I would unknowingly close my mouth, then wake up every few seconds because I could not breathe through the cotton packed in my nostrils, and I stayed up all night. Seriously, I wondered if this was what a panic disorder felt like, if this was what it felt like to suffocate, and I thought I was going to die. Daytime life with the nostrils blocked had been a piece of cake. After the cotton was removed the next day, it felt like a window had opened in my nose. Since the septum was corrected too, breathing became completely better. On day 7, I had the splint removed and saw the director, and he prescribed more antibiotics for my still-chubby nose. Because I asked so many questions, I had this hallucination that the director would step backward when he saw my face.. Post-op days 1, 2, 3... through 10 were the life of a mentally unstable person. Especially after the splint was removed, other people were posting photos of straight, well-defined nasal bridges, but I looked in the mirror and felt devastated, wondering why I looked like I had a clown nose. I comforted myself, saying it was because my skin is thick and because a lot had been removed, including silicone, Medpor, and scar tissue. Then I looked in the mirror again and felt devastated again. The mentally unstable period of looking in the mirror every time I turn around is still ongoing. Meanwhile, my nose is gradually getting smaller. At a glance, it looks like a small, pretty nose. And I feel a little better knowing it is silicone-free. The director said the base of my nose was asymmetric, so he would at least align the nostrils with cartilage, and the little sprout-like nostrils that resulted from that are so cute. There is still a lot of swelling that needs to go down, so I will end the review here for now and come back after it settles more! As an overall review, I am satisfied with Cobijou! If you want something bold and glamorous, it might not be a good fit, but I wanted to look natural.. And as recommended, his hands were good; the stitching at the columellar incision was art. Seeing the clean sutures after surgery made me feel relieved, like, ah, the surgery must have gone well. On day 10, the columellar scar is only slightly red haha I just hope the nostrils, which were the problem, maintain this sprout-like shape they have now.
