My first surgery in 2014 involved hump removal, 3.5 mm silicone, septal extension, and the use of septal cartilage. After about 2 years, I developed contracture. I did not really have major reactions such as inflammation, pain, or odor, but at some point the length gradually started getting shorter. I also have fairly severe facial asymmetry, and right after the first surgery my nose was not crooked and stood straight, but after the contracture, the symptom of it deviating to one side became worse. Even though 2 years had passed, when I contacted the same hospital, they immediately suggested revision surgery, charged only the anesthesia fee, and performed the revision. They did not do a lot of new procedures; after opening it up, they removed all the areas that had inflammation and proceeded in the direction of only lengthening the nose. I should have removed the silicone then, but I was in too much of a hurry and went ahead without even looking into other hospitals. Then almost 8 years passed, and the deviation of the columella to one side became very noticeable. One nostril kept looking like it was being pulled upward, which really did not look good, so although I did not have pain or anything, this time I went around for consultations after deciding to remove the silicone completely and correct only the nasal tip.
At the first consultation clinic, they focused on revising it with custom silicone after removing the existing silicone, and they also suggested doing functional surgery at the same time. Since I first of all had an aversion to using silicone again, I passed. At the second consultation clinic, they talked about removing the silicone and correcting only the nasal tip again, and said they would reuse the cartilage that had already been used. At the third consultation clinic, they described a major surgical plan including silicone removal, moving the septum to correct septal deviation, reinforcement with costal cartilage, alar cartilage grafting, and nasal lengthening, and the cost was enormous accordingly.
In the end, I proceeded with removal at the second consultation clinic. The director took photos personally, consulted with me again about the surgical direction, and then performed the surgery. They are a place that does not use a splint or nasal cotton, and it was done under sedation and local anesthesia. It did not hurt much, and afterward there was almost no swelling. However, as I remember it, they had a really hard time taking out the silicone, so I am going to ask once when I have the stitches removed why that was. During the surgery, they showed me the removed silicone and told me to touch it lol And from the nasal bridge to the dorsum, they squeezed it out with a lot of force, as if pushing it out. When I said it hurt and asked why they kept doing this (I was under the effects of sedation, but because it was local anesthesia I could still talk), my memory is hazy, but I think they said they had to do this so there would be no inflammation or shape changes. After it ended, I slept for a bit in the recovery room, then took the subway home. If it had been full sedation anesthesia, it probably would have been hard as the anesthesia wore off, but since it was not like that, it was manageable.
On day 3, I went in for disinfection and got a swelling injection. At that time, the inside of my nose felt a bit uncomfortable, so I wondered if they would remove all the foreign material inside, but they only did a little, so that was slightly disappointing. While taping my nose, the director said the swelling would gradually go down and the nasal tip would come down more too. Of course I know that, but right now the nasal tip is lifted so much that I am a bit worried.
Today, day 4, the inside of my nose is so dry that it feels stuffy. I originally had severe rhinitis, so I always lived with Vaseline applied inside my nose, but since I have not been able to apply Vaseline these past few days, it feels even drier. They told me not to touch the inside of my nose, but I shone a flashlight inside and gently took out the foreign material hanging in my nose with tweezers. I feel like I can live now lol There is almost no swelling, but when will the nasal tip come down...
