<Natural nose - surgery - removal>****The hospital is still in my recovery period, so if you are very curious I can tell you, but please use it only as a reference. Even if you do not ask, if you search my ID you can probably infer it****
1. Surgery
- My rhinitis, which had developed quite a while ago, progressed into drug-induced rhinitis from Otrivin, and my airway was blocked, so I was in a situation where physical removal was needed - Since my nose was small and low, I decided to do it at the same time
2. Nose condition
- My first surgery was in December 2019, and the removal surgery was done after completing a full year - During the first surgery, they said they would use 4? 5? mm silicone + septal cartilage + ear cartilage if there was not enough, and went into surgery. For the rhinitis, I was told it would be correction of deviated septum and inferior turbinate removal - After removing the nasal packing, breathing was comfortable for a day or two, but I continued to be in a situation where I actually could not breathe as well as before surgery, so I visited a local ENT. They found a hematoma and wrote a referral to a university hospital - At the university hospital, I had hematoma removal and revision surgery for the deviated septum - For about three months, I could breathe well, but after that, even if it opened up in the morning, it would get blocked in the evening, so breathing became uncomfortable again by about 50%. - After the revision surgery for the deviated septum, inflammation suddenly developed inside my left nostril. I visited an ENT - they said it was highly likely due to the cosmetic surgery. It subsided after antibiotics
3. Reason for removal
- The nasal tip looked white from before the splint was removed after surgery, but there was no follow-up treatment - After that, it progressed into discoloration. The show-through area widened and became white - The nasal tip was hard, and if it sometimes got bumped, it would sting and reverberate - My inner eye corners looked pulled inward + my midface looked longer from the columella being lowered, making me look much older - I started wanting to do combat sports or ground-based exercises - The pain of not being able to breathe well (Whether it would improve was a matter of luck)
4. Removal consultation
- Open approach - Will the nose become upturned? They would reposition the cartilage. My natural nose was upturned, and my operated nose was one where the columella had been lowered, so it would be more upturned than now, but not more than my natural nose - Risk of saddle nose? Since I had rhinitis surgery and then revision surgery again, they could not know the state of my septal cartilage, so if any risk arose, they would use the septal support that was inside to prevent it as much as possible. - After removing the silicone, if it is rasped or spreads, they would properly handle the rest by rasping it or crushing it and putting it back in - Since there was no particular inflammation, they did not remove the capsule - They would remove everything as much as possible, but if after opening it, the septal cartilage was too weak and there was even a small possibility of collapse, they would graft septal cartilage alongside the original septal cartilage.
4. After
** Once they actually removed it, silicone, septal cartilage, ear cartilage fragments, donated rib cartilage, and ear cartilage spreader grafting? (I did not hear the details on this) came out. Because the donated rib cartilage had made the septal cartilage thinner, they reinforced the septum.
- Surprisingly, there was no bruising. Swelling was only around the nose, and the rest was fine. Nosebleeding only lasted until the evening of the first day. I only did ice packs briefly in the evening and stopped - As soon as I came out of the recovery room after removal, I could breathe very freely, and I still breathe well now. They showed me right away that I could make a pig nose - Pain was only a little intense until the evening and then disappeared. Is it okay for it to hurt this little? - After the first day, the swelling on the nasal bridge went down quickly and almost completely disappeared - So far, the nose shape seems close to my natural nose - The nasal dorsum seems slightly sunken, or maybe not - Surgery was on Monday, and until stitch removal on Saturday, I did not go for the midweek visit. Even until the day I went to have stitches removed, my nose was runny, but it was not as bad as during the first surgery. - The show-through and discoloration are a little better, but I think this is something I just have to live with

- After day 7 passed, I actually felt like my nose became a bit firmer. Pushing the nose upward feels soft, but when doing half of a pig nose it feels a little hard, so I have not touched it. - After day 7, there is actually a little pain. When I touch my nose, it feels like stinging or tightness when a scab is forming
- I blow my nose gently. After eating, my runny nose increases, so if I blow it, it comes out well
- It already looks like my original nose shape, as if all the swelling has gone down, so I wonder whether it will go down more from here.
Lastly,
1. Assuming the current condition continues, I feel like the functional issues were actually worsened by the donated rib cartilage and spreader grafting. It feels like that part was removed and came back. 2. As for the nasal bridge or nose shape, if the current condition is maintained, I cannot say there is no sense of loss, but I rather feel that I removed it safely, so I think I can accept it. 3. The nasal tip is still swollen, so it actually seems to make the nasal bridge look even more absent... From the side, it looks similar to my natural nose
