The clinic said that my eyes have a deep epicanthic fold and a wide distance between the eyes, so if I only did double eyelid surgery, my eyes might actually look more stuffy, and they strongly recommended an inner canthoplasty. They said that if I do the inner canthoplasty together, the front corners of the eyes will open up clearly and the in-out or semi-out line will fall neatly. They also said that if you widen it too much, the pink flesh (caruncle) can show and make the eyes look harsh, so they would make it natural with only 1 to 2 mm according to my eye proportions
