There is no separate consultation reservation; you just go and wait. My revision eye surgery got rejected... He poked the skin a few times and said my skin tissue was too thick, so even if I did revision surgery the asymmetry would not change... He basically meant I should wait until I get older and the skin sags, and then do it then? I get it, but I came all that way because I was desperate, and hearing him say, "Just live like that" really irritated me. He could have at least said he was not confident because he only handles easy cases, haha. Since I got rejected, I do not know the price... I just entered something random.
