I remember them saying that if you put on anesthetic cream, it swells up and makes it hard to shape things carefully, so they do not apply it and instead use a very thin needle. It did not hurt and was tolerable. I did not like the double-lip look, so I asked them not to put anything in the middle, but they listened to my opinion well and suggested that maybe putting in just a tiny amount would be better in case it looked odd, which I appreciated.
