I had a philtrum reduction and nasal-tip revision together, and also had alar elevation. At first, I was only thinking of revising the nasal tip, but as I researched different things, I thought doing the philtrum and alae together would make the overall look better, so I proceeded with all of them. Before deciding on surgery, I really put a lot of effort into researching. I spent a long time looking for reviews and personally visited about three clinics for consultations. Each clinic had somewhat different surgical methods and explanations, so I became even more deliberative after the consultations. But when I consulted at [Clinic A], I liked the incision method, which was what mattered most to me. I asked one by one about scars and recovery, and they explained instead of brushing anything aside, which was reassuring. I also showed the style of nose I wanted; rather than saying they would make it exactly like the photo, they recommended a similar direction suited to my face, which I liked. Since it was a revision rhinoplasty, I was secretly worried that my nose might look odd again, but once I had a general direction during the consultation, it was easier to decide. It has not completely settled after surgery yet, but my nasal tip feels more refined than before. Since the alae were adjusted too, the balance looks better from the side than before. My philtrum also changed naturally without feeling overly shortened, so for now I am satisfied and watching how it progresses. Since I decided after consulting multiple places, I think I chose rather carefully.
