That's really different for every person and every case, so maybe that's why people look for a director with lots of experience who has operated on many different cases?? If you're scared of going as far as repositioning, couldn't you just do under-eye fat removal?? I don't know that well either, but I'm saying this because I saw on a YouTube channel that removal alone can be effective enough. Look up a lot of YouTube videos and things like that.
