I have a contracted nose using silicone and ear cartilage, so the nasal tip is pulled upward and the columella has also receded inward.
They suggested that since my nasal bridge is not that low, I should remove the silicone and use autologous costal cartilage for the nasal tip. They said the step-off between the nasal bridge and nasal tip could be covered with fascia harvested together while harvesting the rib cartilage, and that osteotomy would raise the nasal bridge by about 1 mm, so the shape would become pretty.
I agree, but I'm a bit worried about the osteotomy (because breathing might become difficult), and if the fascia were to be completely absorbed, there is a possibility of a saddle nose, so I haven't been able to decide readily.
The director was very kind and talked well about the suggested plan and the parts I was concerned about. There were an enormous number of before-and-after photos, and the nose shapes were designed so beautifully that it gave me trust. The consultation manager also wasn't very young, so during the consultation she shared various things, which was good.
The patients' nose shapes are so pretty and there is nothing more to ask for, but I think I just need to think a bit more about whether the surgical method and materials suit me.
