I had surgery on both cheeks to create dimples about 3 years ago. For a few months after the surgery, the hollows in my cheeks were so deep even when I wasn't making an expression that I worried, but over time they became natural. Three years later, they are only slightly visible when I keep my mouth closed and smile. It doesn't feel like "I have dimples!" exactly, but rather natural, which is fine.
