They said to lift the tip of the nose with costal cartilage, correct the bulbous nose by removing fat, slightly smooth the hump, and do osteotomy. They said not to use an implant and not to put anything into the bridge. They said just moving the tip forward would already make a dramatic difference, and that they would use costal cartilage, not septal cartilage or ear cartilage. That way, they said, they could bring the columella forward in an ideal way. They said it would be difficult to create the ideal look with septal cartilage or ear cartilage. They said ear cartilage would just be placed on the tip like a shield graft. I’m planning to keep it on the list of possible surgeries.
