I had surgery 15 years ago, and it was done without an open approach and came out naturally and well. The downside is that if I don't say anything, nobody knows. I don't know whether it's still like that now, but during the consultation you only talk with the manager, and the doctor isn't there from start to finish during the surgery. I can't remember the cost because it was so long ago
