Diagnosis: R - Release the existing adhesion and lower the crease by creating a double crease (low semi-out), with upper-eyelid fat repositioning (this may be confirmed during surgery). L - Release the existing adhesion, match the symmetry with the right side, and reposition the upper-eyelid fat (this may be confirmed during surgery). - Reason: - Because the last surgery was non-incisional ptosis correction, releasing the existing adhesion should not be very difficult. If an incision is needed, the eyes will not close. - Also, the starting point of the right inner corner is not too high, and the incision line extends into the suspected area, so creating an in-out crease may cause a double fold. Therefore, it seems best to maintain the height at the inner corner and lower only the height of the crease toward the back. - It does not seem necessary to lower the left side further. However, if only one side is operated on, comparison is impossible because one side receives anesthetic injections and swells during the procedure, so both sides must be done. - With protruding eyes, the fat sits farther back. Therefore, the condition of the fat on the left needs to be checked by opening it to determine whether repositioning is necessary.
