The director does what you want. When I asked, "Can you do it like this?" they said it was possible, even though other hospitals had said the issue was difficult or impossible. However, there was no explanation of the side effects and problems that could occur if it was done the way I wanted, and because of those side effects I ended up getting revision surgery at another hospital.
