The doctor is not exactly the super kind, gentle type, but they do know how much filler is already in the upper and lower lips separately and which areas need more. I had shown a reference photo, and they understood well how to make my current lips look like the lips in the photo. They did not spend a huge amount of time explaining the design. I was worried it would hurt during the procedure, but they are not the type to indulge whining
