I went because of an event, and it was cheap. But when I consulted at other hospitals, they never brought up thread lifting, yet here they kept saying it was essential and that if I didn't do it together, it would definitely sag. (In the end I had it done elsewhere and it didn't sag..) If you don't add the threads, it does seem cheap
