Maybe they are the type who doesn't like doing aggressive procedures? If your expectations for the surgery result are high, they seem a bit reluctant. I got the impression they were pursuing a natural look. Back then I didn't trust that attitude, but looking back now, I think natural would be fine. For reference, this is an old consultation experience. . Around 2014 / fat grafting
