The director was kind, and I emphasized that I definitely wanted it done thin, so they wrote that in the chart and marked it with a star! And on the day of surgery, before anesthesia, a nurse?! asked me if I had any photos to show the director! I did not show any separate photos, but if you are getting surgery, it might be good to bring one on the day of surgery too.
