It seems like they prefer a firm and safe path. I went there wanting in-out back canthoplasty, but I was diagnosed with a thin in-line and upper front canthoplasty and had the surgery. I do feel some tension in the fixation, and the line feels a bit uneven. Also, even after the surgery, I still linger over the line I originally wanted. If your preferred direction matches the doctor's, I think you could be satisfied.
