I had surgery as a revision procedure. I went because the contracture was severe, and for me the tip of my nose dropped again, but the contracture did not recur,, The hospital staff have been the same people for years and years and never changed! Even after getting nose surgery in many different places, I still think they are good at dealing with contracture.
