Using silicone and ear cartilage for a contracture case, the tip is being pulled upward and the columella is also tucked inward. Since the nasal bridge is not that low, they suggested removing the silicone and doing the nasal tip with autologous costal cartilage. The height difference between the bridge and the tip can be covered with fascia harvested together during the rib harvest, and if the bridge is osteotomized it will rise by about 1 mm, so the shape should look prettier. I agree, but I am a little worried about the osteotomy (because I am afraid breathing might get difficult), and if the fascia gets fully absorbed there is a possibility of saddle nose, so I still cannot make the decision easily. The director was very kind and explained the suggested points and my concerns very well. There are an incredible number of before-and-after photos, and the nose shapes are so beautifully done that I trust them. The consultant at the office was not very young either, so she talked about all sorts of things during the consultation, which was nice. The patients' nose shapes are so pretty and there is nothing more to ask for, but I think I just need to think a bit more about whether the surgical method and materials suit me.
