For my first surgery, I had the nasal tip done with ear cartilage and the columella with donated costal cartilage, but it was too pointy, and over time the nasal tip cartilage also started showing through. So this week, I’m having surgery to lower the nasal tip and use temporal fascia to cover the show-through. But the doctor said my columella is slightly retracted, so they suggested using new donated costal cartilage to lower the columella and correct my bulbous nose. Maybe it’s because I don’t really understand how important the columella is…. Would it be better for me to lengthen the columella? I hadn’t thought much about it, but the more I keep looking, the columella seems like it hasn’t come down enough, so from a 45-degree angle and such, it kind of looks like only my nose is sticking out too pointily..? I want a revision with an extremely natural feel, so what do you think would be best?
